The information collected on this form is recorded in a computerized file by the Training Department of the Institut Curie Hospital (enseignement.dd@curie.fr) for the submission of an internship application for external physicians. The data collected will be communicated to the following recipients only: Training Department of the Institut Curie Hospital, Heads of the department concerned by the request. The data is kept for the duration of the training if the request is accepted, 3 months otherwise. You can access your data, rectify them, request their deletion or exercise your right to limit the processing of your data. You can withdraw your consent to the processing of your data at any time. You can also object to the processing of your data. In these cases, your request will be closed and your data will be deleted. Consult the cnil.fr website for more information on your rights.
